Little Ethan calling Gertrude “Mom” nearly raises Ethan’s suspicions, but Theresa manages to talk her way out of trouble and privately urges her son to keep her identity secret. Ethan lays down rules for the new nanny while Gertrude carefully asks about Theresa. At the hospital, the drunken Eve performs Julian’s surgery as Vincent interferes grotesquely, leaving Rebecca horrified by the result. Elsewhere, Pretty bribes a nurse and collapses just as Fancy confronts her about Luis, setting up another scheme.
